Pay Bill

Patients will incur a facility fee for the surgical services provided by Sage Surgical Center, an anesthesia fee for anesthesia services, and a professional fee from their surgeon. Additional charges may apply for services provided by ancillary providers, such as laboratory, pathology, or pharmacy, when medically necessary.

Payment in full, an approved payment plan, or insurance assignment is required before surgery. Patients should be prepared to meet any personal financial obligations.

Please bring your insurance card or a copy of both sides on the day of surgery. If needed, you may be asked to sign an assignment of benefits form to process an insurance claim.

As a courtesy, we will submit all insurance forms for patients who have prior authorization.
